Job Descriptions

Description

  • Provide service to individuals in their own homes and communities who need assistance caring for themselves as a result of old age, sickness, disability, and/or other conditions. Personal care may include assistance with activities of daily living, housekeeping, laundry, meal preparation, transportation, companionship, and respite care.
  • Ensure services are delivered in a caring and respectful manner in accordance with Agency policies and industry standards.

Reporting Relationship

  • Reports to Supervisor.

Responsibilities / Activities

Assist with activities of daily living and personal care, including:

  • Bathing
  • Shaving
  • Ambulation
  • Mouth Care
  • Dressing
  • Exercise
  • Hair Care
  • Feeding
  • Toileting
  • Nail Care
  • Positioning
  • Medication Reminding
  • Skin Care
  • Transferring
  • Vital Signs and Blood Pressure Monitoring
  • Ensure the client's safety and security by supervising the home environment.
  • Teach and perform meal planning and preparation.
  • Perform routine housekeeping duties, including:
    • Making and changing beds
    • Dusting
    • Vacuuming
    • Washing floors
    • Cleaning kitchens and bathrooms
    • Laundry
  • Provide companionship through social interaction, conversation, emotional reassurance, and activities that stimulate the mind.
  • Provide respite care for families according to care plans.
  • Perform or assist with shopping and errands, including handling client funds when authorized.
  • Assist clients in following physician-directed special diet plans.
  • Escort clients to medical appointments, shopping, errands, and outings as outlined in the care plan.
  • Assist clients with communication by writing, typing correspondence, or researching information.
  • Participate as a member of the Care Team by providing suggestions and feedback.
  • Ensure services comply with Agency policies, procedures, and standards.
  • Monitor supplies and resources.
  • Evaluate the program and recommend improvements when appropriate.
  • Follow the written care plan.
  • Carry out duties assigned by the Supervisor.
  • Report unsafe conditions in the client's environment to the Supervisor.
  • Report behavioral, physical, cognitive, or living arrangement changes to the Supervisor.
  • Maintain accurate records of daily activities, observations, and service hours.
  • Attend orientation, in-service training, and staff meetings.
  • Develop and maintain positive working relationships with clients, families, and co-workers.
  • Make decisions and solve problems effectively.
  • Communicate regularly with Supervisors and co-workers.
  • Observe, receive, and gather information from appropriate sources.
  • Perform other duties as assigned.

Required Knowledge

  • Knowledge of personal care and home management skills.
  • Knowledge of client care principles, quality standards, and client satisfaction.
  • Knowledge of the English language.
  • Knowledge of emergency first aid and CPR.
  • Knowledge of clerical procedures, record keeping, and completing forms.

Qualifications / Education

  • Certification in Personal Care, if applicable.
  • Current driver's license.
  • Proper vehicle insurance coverage.

Training / Experience

  • Related experience may be required.
  • On-the-job training will be provided for new activities.
